The Medical Fee-For-Service rates for Addiction and Mental Health treatment:

$450.00 – For the Initial Evaluation

$175.00 – For a 25-minute follow-up visit

$350.00 – For a 40-50-minute follow-up visit

$475.00 – For a 60-90-minute, follow-up visit

The Addiction & Mental Health Counseling (Psychotherapy) rates:

$350.00 – For the Initial Evaluation

$150.00 – For a 25-minute follow-up visit

$200.00 – For a 40-50-minute follow-up visit

$300.00 – For a 60 follow-up visit
